Shedding pounds with GLP-1s? Beware of this possible side effect

The rise of GLP-1s (glucagon-like peptides) has revolutionized obesity management. These medications help people shed pounds quickly by enhancing insulin secretion, suppressing appetite and slowing digestion. However, rapid weight loss can outpace the skin’s ability to retract, leading to a common skin concern: excess skin.
One study suggests that individuals using these weight loss treatments may be more likely to develop loose, sagging skin. This has led some to explore plastic surgery for cosmetic correction. While the speed of weight loss plays a role, genetics also significantly influence skin elasticity.
“GLP-1s, like semaglutide and tirzepatide, are powerful weight management tools, but rapid weight loss can contribute to the loss of skin elasticity,” explains Amy Paulus, a weight management nurse practitioner and bariatric coordinator at Advocate Health Care. “Additionally, genetic factors are also crucial – some individuals naturally have more elastic skin, while others are more prone to excess skin, even with gradual weight loss.”
Although some degree of excess skin is inevitable with significant weight loss, Paulus advises her patients to take proactive steps to support skin integrity:
- Maintain a balanced, protein-rich diet. Adequate protein intake is essential for collagen production and skin elasticity.
- Stay hydrated. Drinking plenty of water supports skin health and may improve its ability to retract.
- Incorporate strength training. Regular exercise, especially strength training, can help build muscle that can provide structural support beneath the skin, reducing the appearance of excess skin.
- Aim for gradual weight loss. Losing weight at a steady rate of 1 to 2 pounds per week allows the skin more time to adjust, potentially minimizing excess, saggy skin.
“Having loose skin is a good problem to have,” Paulus says. “It’s a sign of how far you’ve come in improving your overall health, lowering cardiovascular risk, and regaining mobility and confidence. These are all major wins.”
